Eliquis (apixaban) is a popular brand-name oral blood thinner medication that is used for the prevention of issues such as blood clots and stroke in patients who have nonvalvular AFib, deep vein thrombosis, or pulmonary embolism. It is also used to lower the risk of developing DVT blood clots following hip or knee replacement procedures. This medication usually comes in an oral formulation that is to be taken twice per day.
This medication is beneficial to several groups of patients.
| Brand Name | Eliquis |

|
| Mechanism of Action | Prevents clot formation by targeting factor Xa and reducing thrombin generation |
| Dosage Forms | Oral tablets |
| Strengths | 2.5 mg or 5 mg (standard strengths) Note: 0.15 mg strength formulations may be available for the treatment of some pediatric patients |
| Half-life | 8-15 hours (usually about 12 hours) |

The dosage of Eliquis depends on the indication and the specific patient.
The standard Eliquis dosage for most adults with nonvalvular atrial fibrillation is 5 mg taken twice each day. Some patients may require a reduced dose of 2.5 mg twice daily if they meet at least two of the following criteria:
To help reduce the risk of blood clots following hip or knee replacement surgery, treatment is typically started with 2.5 mg taken twice daily beginning 12 to 24 hours after the procedure. Therapy is generally continued for 35 days after hip replacement surgery or 12 days after knee replacement surgery.
Patients using this medication for this treatment usually take 10 mg twice daily for the first seven days. They usually then continue with 5 mg twice daily following this.
The recommended dose is 2.5 mg twice daily after completing at least six months of treatment.
Dosing is based on the child’s weight and is started after at least five days of initial anticoagulant treatment. The healthcare provider will determine the appropriate dose and treatment duration.
Eliquis is not recommended to be taken by children weighing less than 2.6 kg (or 5.7 lbs). Patients using this medication should take it exactly as they are instructed to do so by their healthcare provider. Doses of this medication are usually taken twice per day with 12 hours between each dose. It does not matter whether or not this medication is taken on an empty or full stomach.
Missed doses of Eliquis should be taken as soon as they are remembered – as long as it is on the same day. However, doses should never be doubled to make up for any doses that were missed. Patients should skip their missed dose if it has been more than six hours since the time it was originally supposed to be taken. Patients who overdose on this medication should immediately call their local poison control center or emergency medical services.
Eliquis has two different black box warnings assigned to it by the FDA. The first warning is about premature discontinuation of the medication. Prematurely stopping this medication – or other oral anticoagulants – can increase the patient’s risk of experiencing thrombotic events. Patients who need to stop Eliquis should talk to their doctor first and will likely be switched to another anticoagulant to reduce this risk if the medication is being stopped for a reason other than pathological bleeding or the end of treatment.
The second boxed warning for this medication is that an epidural or spinal hematoma can sometimes occur in patients using this medication who are also taking neuraxial anesthesia or who are undergoing spinal puncture. The development of these issues can cause serious long-term paralysis.
Eliquis can also increase the risk of serious or life-threatening bleeding. Patients should contact a healthcare provider right away if they experience unusual bleeding, bruising, or signs of blood loss. A reversal agent is available in emergency situations to help counteract Eliquis’s blood-thinning effects. Eliquis is also generally not recommended for people with artificial or prosthetic heart valves.
This medication may cause an increased risk of blood clots in patients with triple-positive antiphospholipid syndrome and is also not recommended for patients in this group. Patients should also not use Eliquis if they are currently experiencing any abnormal bleeding or have an allergy to any of the ingredients of the medication. Advise a doctor of all medications and conditions in personal or family medical history before taking the medication.
Patients should not take Eliquis if they have active pathological bleeding or a serious hypersensitivity reaction to the medication or any of its contents. Signs of allergic reactions include those such as swelling or rash, troubles breathing, and other severe symptoms that indicate the need for immediate medical attention.
There are still no significant studies completed that focus on how this medication impacts pregnant women. Patients using this medication while pregnant or breastfeeding should weigh their benefits versus the potential risks before deciding to take this medication at the same time as pregnancy or breastfeeding.
Apixaban affects blood clotting and can make bleeding more likely. Common signs may include:
The most common side effects for children include those such as heavier menstrual bleeding, headache, or vomiting. Additional side effects that some people may experience include:
Many of the common side effects may resolve on their own over time and can be more common when doses are started or increased. Let a doctor know if any of these effects worsen, are severe, or do not go away on their own.
Patients may experience more severe side effects in rare instances while taking Elquis. This may include effects such as increased risk of blood clots after stopping treatment. Stopping Eliquis without medical guidance can increase the risk of developing dangerous blood clots. These clots may lead to a stroke or other serious complications. Do not discontinue the medication without talking to a healthcare provider first.
Eliquis can also sometimes cause severe or life-threatening bleeding. Eliquis reduces the blood’s ability to clot and can cause serious bleeding that may require emergency medical attention. Seek immediate care if any of the following symptoms are experienced:
Eliquis may also cause serious spinal or epidural blood clots (hematomas) in rare cases. People who receive spinal injections, epidural anesthesia, or spinal punctures while taking Eliquis may develop a blood clot around the spinal cord. This can lead to the patient experiencing permanent nerve damage or paralysis. Patients should get immediate medical attention if they develop:
Seek immediate medical attention if any serious side effects are experienced.
Eliquis increases the patient’s bleeding risk. This is why patients need to talk to their provider before combining this medication with the use of:
A provider’s review will determine whether or not these medications should be used together. Let a doctor know any medications or supplements being taken before starting the medication.
Eliquis should be kept in a safe place with temperatures between 68°F and 77°F or 20°C and 25°C (room temperature). This medication may be used until the expiration date listed on the medication as long as it is kept in these safe storage conditions. This medication will be carefully packaged when shipped to patients’ homes in the U.S. to make sure these conditions are maintained.
