Flovent HFA (fluticasone propionate) is an anti-inflammatory synthetic trifluorinated corticosteroid that is used for patients with asthma and makes it easier for them to breathe. This controller medication comes in the form of an inhalation aerosol that is prescribed for long-term maintenance treatment. It is used for the prevention of symptoms such as shortness of breath and wheezing, not for quick relief.
Patients taking Flovent HFA may experience a range of different benefits.
| Brand Name | Flovent HFA |
| Generic Name | Fluticasone Propionate |
| Drug Class | Inhaled corticosteroid |
| Indication | Asthma controller medication |
| Mechanism of Action | Decreases inflammation in the lungs/airways for easier breathing |
| Dosage Forms | Inhaler |
| Strengths |
|
| Half-life | About 10–12 hours |
| Manufacturer | GlaxoSmithKline |
Flovent HFA comes in strengths of 44 mcg and 110 mcg. It also comes in 220 mcg strength. Patients who are aged 4-11 years old usually are given 88 mcg twice per day with doses spaced 12 hours apart from each other. Young children often require a valved holding chamber (or spacer) and face mask to ensure the medication reaches the lungs effectively. Patients aged 12 years and older may be given this same dose. However, this dose may also be increased if a healthcare provider determines it to be necessary to do so. Doses should not exceed 880 mcg administered twice per day for patients aged 12 years or older. Children taking inhaled corticosteroids must also have their growth monitored routinely by a healthcare professional. Physicians should always titrate patients to the lowest effective dosage once asthma stability is achieved to minimize systemic side effects.
Flovent HFA inhalers should be primed before first use or as needed. This can be done by removing the cap and shaking the inhaler for 5 seconds. It will then need to be sprayed once into the air away from the patient’s face and avoiding their eyes. This should be repeated three more times. The inhaler should be primed again with one spray if it has not been used for more than 7 days or if it has been dropped.
Patients can use their primed inhaler by:
Some patients have reported developing throat and mouth infections like Candida albicans while taking this medication. Patients who develop this should be closely monitored. This issue is also less likely to occur for patients who rinse their mouth out with water without swallowing after taking their daily inhalations of the medication. Patients should always rinse their mouth with water and spit it out without swallowing to effectively reduce the risk of oral thrush.
Use of this medication can also worsen any existing infections that patients have. This includes parasitic infections and fungal infections. It also includes bacterial and viral infections. It may also worsen conditions such as ocular herpes simplex and tuberculosis. Patients taking this medication while suffering from chickenpox or measles are also at an increased risk of experiencing serious effects and potential fatalities.
Cushing syndrome or adrenal suppression may happen in some instances where patients are either taking high doses of the medication or if the patient is taking a regular dose of the medication while extra susceptible. Patients should slowly work their way off of this medication if either of these conditions occur and bone mineral density should be routinely assessed. This monitoring is critical for patients on high doses or those with risk factors since long-term corticosteroid use is linked to significant bone density reduction. Serious adrenal insufficiency can occur when transitioning from oral steroids to Flovent and a strictly supervised medical tapering schedule will be required.
Pediatric patients taking this medication should have their growth monitored. Long-term use of the medication may also cause some patients to develop ocular issues such as glaucoma and cataracts. Patients taking this medication should undergo routine eye examinations to monitor for these conditions-especially if they have a history of increased intraocular pressure.
Patients should let their doctors know of any medications or supplements they are taking before starting Flovent HFA. They should also make sure they are aware of any conditions they or a family member has had that may impact how this drug affects them. Users should also be aware that Flovent is not for quick relief and is contraindicated for the primary treatment of status asthmaticus or acute asthma episodes requiring intensive measures.
Patients taking this medication may experience one or more of the reported common side effects. However, it is not a guarantee that all patients will necessarily experience any of these. These include effects such as:
These side effects may resolve on their own and are not always necessarily serious. Patients whose side effects become severe or will not go away on their own should let their doctor know immediately-as it could indicate more serious issues.
Patients also occasionally experience serious side effects while taking this medication in much rarer instances. These effects are more likely to occur in patients who have been on the medication in high doses for long periods of time. This includes experiencing effects such as:
Patients should get immediate medical attention if they experience any of the serious side effects listed above or signs indicating any of these issues.
Flovent HFA has a known drug interaction with strong cytochrome P450 3A4 inhibitors. This includes medications like ritonavir and ketoconazole. It also includes medications such as:
Use of these medications together may increase the patient’s risk of experiencing systemic corticosteroid effects. Consult a healthcare provider prior to taking the medications and let them know of any other medications or supplements that are being taken.
Flovent HFA should be stored at room temperature (68°F to 77°F or 20°C to 25°C). It may be kept in ranges between 59°F and 86°F (15°C to 30°C) for short-term periods of time. This device should also be stored with the mouthpiece facing down. The pressurized canister must also be protected from freezing and direct sunlight since exposure to heat above 120°F (49°C) may cause it to burst.
The inhaler for this medication should be replaced whenever the counter reaches “000.” The device must be discarded immediately even if it still sprays. This is the case even if the inhaler still sprays since the dosing on the device may no longer be accurate and the spray is no longer guaranteed after this point. The canister that the medication comes in is pressurized and should never be punctured or exposed to too much heat. This may cause the canister to burst. The dark orange actuator that comes with Flovent HFA should also be the only one that is used with it. It should not be mixed with any other inhalers or canisters. This medication is carefully packed and shipped to patients’ homes to secure its quality when it is ordered from Canada to the U.S.
