Rybelsus (semaglutide) is a type of orally taken prescription medication designed for daily use to help adult patients with type 2 diabetes management. Patients using this medication are instructed to pair it with healthy dietary changes and exercise so that this medication can have its full effects for lowering blood sugar levels and may have cardiovascular benefits. Rybelsus has been proven to lower patients’ A1C levels by significant amounts.

Patients should take this daily tablet medication on an empty stomach upon waking. They may take their pill with no more than 4 oz of plain water. Rybelsus users will then have to wait at least 30 minutes before taking their first food, beverage, or other oral medications. This is the same duration of time that should be waited by the patient before they take any other oral medications. Pills of this medication should never be split or crushed (unless the patient is specifically told to do so by their doctor or healthcare provider).
The starting dose of this medication that patients are usually prescribed is 3 mg per day. This is the dose they should take for at least the first 30 days they are on the medication and it is only meant for initiation/tolerability. This dose is usually increased to 7 mg per day after this period of time. Patients may be kept on this dose for the remainder of their treatment or their doctor may increase their dose to 14 mg per day after at least 30 days on the 7 mg dose of the medication if they determine additional blood sugar control is needed.
Rybelsus carries a boxed warning due to a potential risk of developing thyroid C-cell tumors. This is a finding that appeared in rodent studies with the medication and includes potentially causing issues such as medullary thyroid carcinoma. Rybelsus should not be prescribed to patients with a personal or family history of medullary thyroid carcinoma due to safety concerns. The same goes for those with a history of MEN 2.
Patients are also told to avoid taking this medication if they have any serious hypersensitivity reactions to semaglutide or any other of the ingredients that are used for the creation of Rybelsus.
Patients should also be cautious about taking Rybelsus if:
This medication is contraindicated or should be discontinued due to potential fetal harm in patients who are pregnant or those who are breastfeeding. Patients should stop taking this medication at least two months before any planned pregnancy since this medication also has a long washout period.
Patients should inform their doctor of any pre-existing conditions that they or their family have had before they start this medication. They should also make their doctor aware of any other medications or supplements that they are currently taking before starting Rybelsus.
Patients taking this medication may experience one or more of several different side effects. The most commonly experienced ones include those such as:
These side effects are more common for patients when they are starting the medication or during dose increases. These side effects often resolve on their own overtime as the body adjusts to the medication. Patients should let their doctor know if any of these side effects are experienced and do not resolve on their own or if they become severe.

Rybelsus itself has low hypoglycemia risk. However, the risk increases only when combined with insulin or insulin secretagogues. Corticosteroids may also interact with this medication by reducing the effectiveness of Rybelsus due to their causing increased blood sugar. Beta-blockers are also known to interact with this medication due to their ability to mask symptoms of low blood sugar. Rybelsus may also interact with certain diuretics and specific medications like warfarin and levothyroxine. Always consult a healthcare provider for personal advice on combining medications and let them know of any medications that are being taken before starting Rybelsus.
Rybelsus tablets should be stored within their original container – not in a pill organizer or other container. Store the container in a dry area where the temperature is between 68°F and 77°F (the equivalent of 20°C-25°C). The blue cap should be left on the bottle when tablets are not being taken. These tablets should also never be split/broken or chewed. They may be stored this way and used until the expiration date listed on the bottle.
