Trulicity (dulaglutide) is a prescription injectable medication that is used for improving glycemic control in adults and children aged 10 years and older with type 2 diabetes. Trulicity is intended to be used alongside a balanced diet and consistent physical activity as part of a treatment plan for managing blood glucose. Healthcare providers may also use the medication as part of an approach to cardiovascular risk reduction in adults living with type 2 diabetes – specifically when a patient has a history of cardiovascular disease or is considered at elevated risk for developing cardiovascular complications.
Trulicity patients can experience many benefits from taking the medication. These may include benefits such as:
| Brand Name | Trulicity |
| Generic Name | Dulaglutide |
| Drug Class | GLP-1 receptor agonist |
| Indication |
|
| Mechanism of Action | Mimics natural GLP-1 for increased insulin secretion, slows gastric emptying, and reduces glucagon secretion |
| Dosage Forms | Single-dose injection pens |
| Strengths |
|
| Half-life | About 5 days |
| Manufacturer | Eli Lilly and Company |
Adult patients on this medication are generally prescribed a starting dose of 0.75 mg for at least the first four weeks of using the medication. Adult patients may then have their dose increased to 1.5 mg per week to give them additional glycemic control. This dose may then be increased to 3 mg per week after at least four more weeks of use if a doctor determines additional glycemic control is needed. The maximum dose that can be prescribed is 4.5 mg per week. Patients will have to be on the 3 mg per week dose for at least four weeks before this dose can be prescribed. Pediatric patients are usually given the same 0.75 mg starting dose. However, the maximum dose they are usually prescribed is 1.5 mg if necessary (after a minimum of the first four weeks on the medication).
Trulicity comes in the form of single-use injection pens. Patients can give themselves their weekly medication using these pens by:
Patients using this medication with insulin must make sure that both injections are separated – never mixed – and injections in the same body region should not be adjacent. Those who accidentally miss doses of their medication should take the missed dose only when at least 72 hours remain before the next scheduled dose – otherwise the missed dose should be skipped.
Dulaglutide caused dose- and treatment-duration-dependent thyroid C-cell tumors in rats in rodent studies and has indicated the potential of medications like Trulicity causing these tumors (including medullary thyroid carcinoma) in humans. It is still unknown what this study means for use in humans. However, this is the reason that the medication has a black box warning and is not to be used in patients with histories – family or personal – of MTC. The same goes for patients with personal histories of Multiple Endocrine Neoplasia syndrome type 2.
Patients should be extra cautious about using Trulicity with insulin secretagogues or insulin since this may increase their risk of developing hypoglycemia. This can be severe and reducing the dose of either the insulin or an insulin secretagogue – such as a sulfonylurea – may be necessary. may be necessary before starting Trulicity. Rare but serious hypersensitivity reactions may occur in some patients. This requires immediate medical attention and the medication should be avoided by patients with known reactions to the medication or any of its ingredients.
Patients on this medication may experience severe gastrointestinal reactions in some cases. Patients with severe gastroparesis should generally avoid the use of Trulicity. Some of Trulicity’s side effects may lead to volume depletion. Patients experiencing these effects should have their renal function monitored as it could lead to acute kidney injury.
Patients with a history of diabetic retinopathy should be monitored while using this medication as there have been reported instances of diabetic retinopathy complications in cardiovascular outcomes trials. Patients with pre-existing diabetic retinopathy require monitoring for progression. Gallbladder studies are to be done on patients if they are suspected to have cholelithiasis or cholecystitis as this medication can also lead to acute gallbladder disease in some rare instances.
Some surgeries and procedures using general anesthesia or deep sedation can lead to pulmonary aspiration during use of this medication. There have been rare postmarketing reports of pulmonary aspiration and available evidence is insufficient to determine whether fasting changes or temporary drug discontinuation reduce that risk. It is important for healthcare providers to be made aware of any surgeries or procedures ahead of time while using Trulicity.
Current labeling states that human pregnancy data are limited. Animal data indicate potential fetal risk and there are no human data on dulaglutide in breast milk or effects on the breastfed infant. Always let a doctor know before using this medication while pregnant or breastfeeding. Patients should also let a doctor know of any medications or supplements being taken while on this medication.
The side effects that are most often reported by those on Trulicity are those such as:
Not everyone may experience one of the above effects. However, it is important to know them. Patients should talk to their doctor right away if they are experiencing any worsening or persistent side effects.
Serious side effects may also occur on some occasions. This may include effects such as:
Patients experiencing any of the above side effects should seek immediate medical attention.
Trulicity may interact with some oral medications by delaying gastric emptying which can potentially change the rate at which these medications are absorbed. Concomitant use with insulin or sulfonylureas may also increase the risk of hypoglycemia. The patient’s doctor may consider reducing the dose of insulin or insulin secretagogues when initiating Trulicity if it is being used to reduce this risk. Current labeling also recommends monitoring drug levels for oral medicines with a narrow therapeutic index – such as warfarin – when administered with Trulicity. Always talk with a healthcare provider to get personal advice before combining medications.
Trulicity pens are to be stored in the refrigerator at temperatures between 36°F and 46°F (2°C and 8°C) and never frozen. These pens can also be stored at room temperature – as long as it does not exceed 86°F (30°C) – for up to 14 days. The medication should be disposed of if it exceeds this temperature, is left out for more than 14 days, or becomes frozen. The medication should also be stored in its original container and be protected from light until use. These medications are carefully packed with cold packs when shipped to the U.S.

Yes. Trulicity is a prescription-only medication. A licensed healthcare provider must determine whether it is appropriate based on the patient’s needs and medical history and provide a prescription before the medication may be purchased.
Eligible patients may be able to order Trulicity from licensed international pharmacies or prescription fulfillment services, like Duty Free Meds, and have it shipped to their homes in the U.S. A valid prescription from a healthcare provider is required before the medication can be dispensed. However, importing prescription medicines from abroad is subject to U.S. federal law and FDA requirements. Personal importation is not a general right or guarantee. FDA may exercise enforcement discretion in certain circumstances based on factors such as the medication, the patient’s circumstances, and the quantity being imported. The medication must also be used for personal use only.
No. Trulicity is not insulin. It belongs to a class of medications called GLP-1 receptor agonists and helps with insulin release, reduces glucagon secretion, and slows gastric emptying.
Both are types of type 2 diabetes medications from the GLP-1 receptor agonist class. The main difference is that Trulicity contains dulaglutide. Semaglutide is the ingredient that is used in Ozempic – not dulaglutide.
The current list price of Trulicity is about $1,006 in the U.S. Patients may pay around this much if they are not using any coverage in the U.S. or they may find this medication for closer to $295 per month when they order from Canada. Exact prices vary by location, insurance coverage, and available savings programs.
Some patients lose weight while taking Trulicity. However, weight loss is not one of this medication’s approved uses. Any weight reduction is considered a secondary effect of treatment for type 2 diabetes and may vary from person to person.
Trulicity pens should be stored in the refrigerator between 36°F and 46°F (2°C to 8°C). A pen may be kept at room temperature below 86°F (30°C) for up to 14 days before it should be discarded if needed.
Neither medication is considered universally better. Both are effective GLP-1 receptor agonists for managing type 2 diabetes but they differ in their active ingredients, dosing options, and clinical effects. Patients can determine the best option for them by talking with their healthcare provider.
A single Trulicity pen provides one measured dose and is designed to be used for one administration. The pen should be disposed of in an FDA-cleared sharps container after the injection is complete.
