Wegovy Dosage Information

This medication comes in pens with a starting dose of 0.25 mg. This titration dose is required to minimize gastrointestinal side effects. This is the dose patients are instructed to stay on for at least four weeks before they then have this dose increased to 0.5 mg. This dose may then be increased to 1 mg in four more weeks after taking that dose. Some patients may also eventually be prescribed doses of 1.7 mg and/or 2.4 mg. These doses are usually prescribed to patients as their maintenance dose – with at least four weeks in between before upping each dose. The 2.4 mg dose will not necessarily be prescribed to every patient.

How to Use Wegovy

This injectable medication is used on a weekly basis by:

  1. Locating the injection site (this site should be rotated with each dose of the medication)
  2. Preparing the pen by removing the cap and placing the single-use pen firmly against the skin at the site of the injection that was selected
  3. Pressing and holding the pen in place until the injection is complete by following the device’s indicator (this typically takes about 10 seconds)
  4. Do not remove the pen from the skin until the yellow indicator bar stops moving and the injection is complete
  5. Remove the pen from the skin and dispose of it safely in alignment with the instructions and local guidelines once the injection is complete

Each dose of the medication should be given on the same day of each week it is taken. Do not share the pen or needles with other patients.

Precautions & Warnings

Black Box Warning

Wegovy has been issued a boxed warning from the FDA for its potential to cause thyroid tumors in certain patients. This was a finding that appeared in rodent studies. However, there has been no confirmed human causal evidence of this occurring and the human relevance of thyroid tumor risk remains unconfirmed. Patients who have previously been diagnosed with – or have had a family member who was diagnosed with – medullary thyroid carcinoma or Multiple Endocrine Neoplasia type 2 should avoid this treatment. Those who are taking this medication and believe they are experiencing any signs should seek immediate medical attention. This may include signs such as a lump or swelling in the area of their neck. This also applies if the patient develops symptoms such as voice hoarseness or difficulty swallowing.

Other Warnings and Contraindications

GLP-1 medications like Wegovy have led to some rare instances where patients have developed acute pancreatitis. There have also been some uncommon cases in clinical trials where patients have developed acute gallbladder disease.

Patients are at a greater risk of developing hypoglycemia when they are taking Wegovy alongside an insulin or an insulin secretagogue medication. Patients taking any of these medications should let their doctor know prior to taking Wegovy so that any necessary dosing adjustments may be made. The risk of developing hypoglycemia while taking the medication alone is low.

Wegovy patients have also developed acute kidney injury as a result of volume depletion in some instances. This is why patients should have their renal function routinely monitored if they are experiencing any adverse events that may potentially cause them to experience volume depletion. The occurrence of severe gastrointestinal effects is also possible in some rare cases for patients taking Wegovy.

Post-marketing reports have found some cases of patients taking Wegovy developing anaphylactic reactions and angioedema related to hypersensitivity to the medication. These reactions are serious and anyone experiencing any signs of these issues while taking the medication should stop taking the medication and seek immediate medical attention.

Extra monitoring should be done for any patient who has either had a family history of or a history themselves of having diabetic retinopathy. The development of diabetic retinopathy has been mainly observed in patients with pre-existing diabetes and Wegovy has been linked to causing these issues in type 2 diabetes patients. Wegovy may also cause some patients’ heart rates to increase and those taking this medication should keep an eye on their heart rate in regular intervals.

Patients taking this medication who have experienced – or begin to experience – suicidal thoughts or behaviors should be monitored closely, with careful assessment of their response to treatment. Temporary discontinuation may be considered in some cases based on clinical judgment and current guidance for patients who are undergoing general anesthesia or certain surgical procedures due to the potential risk of pulmonary aspiration.

Wegovy may cause fetal harm when used by pregnant women. This medication should stop being taken by the patient for at least two months before any planned pregnancy. This is due to the drug’s long half-life. It should also be stopped if a patient finds out that they have become pregnant. It is not recommended to be used in patients who are breastfeeding.

Side Effects of Wegovy

Side effects that are considered common are reported in 5% or more of patients. This includes side effects such as:

  • Fatigue or low energy
  • Abdominal bloating or fullness
  • Nausea or vomiting
  • Dizziness
  • Indigestion (dyspepsia)
  • Gas (flatulence)
  • Headache
  • Burping (eructation)
  • Diarrhea or constipation
  • Acid reflux (GERD)
  • Low blood sugar in patients with type 2 diabetes
  • Stomach or abdominal pain
  • Stomach flu or gastrointestinal infection

Patients are more likely to experience these side effects when they start their medication and they may go away on their own over time. A doctor should be consulted if these symptoms worsen or do not resolve on their own.

Serious Side Effects

More serious side effects may occur for some patients on rare occasions. This might include effects such as:

  • Serious allergic reactions
  • Vision changes (more common in patients with type 2 diabetes)
  • Gallbladder issues or gallstones
  • Inflammation of the pancreas (pancreatitis)
  • Mood changes or suicidal thoughts/behaviors
  • Increased heart rate
  • Severe stomach problems
  • Kidney issues (usually a result of dehydration while taking this medication)
  • Hypoglycemia (usually a result of combining the medication with insulin or sulfonylureas)

Patients who are experiencing any of these side effects should get medical attention right away.

Drug Interactions:

This medication lowers blood sugar and may cause hypoglycemia when combined with insulin or medications that increase insulin release (such as sulfonylureas). A healthcare provider may lower the dose of insulin or these other medications to reduce this risk. Wegovy may also have an effect on the absorption of some specific oral medications. This is because of the medications’ effects to slow rates of stomach emptying. Extra monitoring should be done on patients combining these medications and possible dosing adjustments. Patients should always inform their healthcare provider of any medications or supplements they are taking before starting Wegovy.

How To Store Wegovy

Injection pens of this medication should be stored in the refrigerator within a temperature range of 36°F and 46°F in its original packaging. These pens may also be stored at temperatures up to 86°F (30°C) for no longer than 28 days if the pen can not be refrigerated. It is important that the pens never become frozen. Patients should store their medication in the carton to protect it from light and dispose of it properly after use. This medication is carefully packed with cold packs when it is shipped to the U.S.
